Use the Patient Portal slip the front desk gave you. The temporary password works once and expires 30 days after it was printed.

10 letters and numbers, e.g. ABCD-EFGH-JK

For a child's record, enter the child's birthdate — you'll be set up as their guardian.

Choose Your Own Password

At least 10 characters. Don't reuse the temporary one.

Already activated? Sign in. Lost your slip or it expired? Ask the front desk to print a new one — they'll check a valid ID first.
